Do Indian Nurses Need IELTS or OET for the NCLEX?

The NCLEX has no English test of its own, but your state board and VisaScreen may. What India-educated nurses need, which tests count, and 2026 RN scores.

Contents5 sections

Short answer: the NCLEX-RN does not ask for an IELTS or OET score. The English test comes from two other places on the road to a U.S. nursing job: your state board of nursing, and the VisaScreen certificate used for an occupational visa. If you were educated in India, plan on needing a test for VisaScreen, and check whether your board wants scores before it lets you sit the exam.

This guide separates those three steps and lists the current VisaScreen minimums for registered nurses. It is general information, not immigration or legal advice. Clesial is an independent NCLEX-RN prep platform and is not affiliated with or endorsed by NCSBN, TruMerit, or any board of nursing.

Is IELTS required for the NCLEX?

Not by the exam itself. NCSBN describes registration as a multistep process that involves your nursing regulatory body (in the U.S., the state board) and Pearson VUE. You apply for licensure with the board first, because that application is what makes you eligible to test. When you register with Pearson VUE, you give your program code, an email address, and payment. An English score is not part of that step. See NCSBN's registration steps.

The catch is the word eligible. Your Authorization to Test arrives only after the board declares you eligible, so a board that wants English scores can hold up your exam date even though the requirement is not an NCLEX rule.

State boards set their own English rules

Each board decides what it needs from internationally educated applicants. Two examples show how the wording differs:

  • Maryland. State law requires RN applicants to show written and oral competency in English, and passing an English exam approved by the U.S. Department of Health and Human Services is one accepted proof (Health Occupations 8-302). The board's regulation ties the passing scores to the federal minimums (COMAR 10.27.01.05). TruMerit's Maryland licensure page lists the countries whose graduates are English exempt for initial RN licensure. India is not on that list.
  • Texas. Under Rule 217.4(a), internationally educated applicants for licensure by examination need an approved English test score unless the board decides that a substantial portion of the nursing program was conducted in English (Texas Board of Nursing meeting materials, April 2025). Whether your program meets that bar is the board's decision.

Other boards may accept a different set of tests or attach other conditions. Choose your board, then read its current requirements on the board's own website rather than a forum summary.

VisaScreen needs an English test for India-educated nurses

Licensure and immigration are separate tracks. Under the federal rule at 8 CFR 212.15, a nurse seeking admission to the United States to work as a nurse generally must present a certificate from an approved credentialing organization. For most nurses that is VisaScreen, issued by TruMerit (formerly CGFNS), which the rule names as an approved organization for nurses.

To get the certificate you need a passing score on an approved English test unless you are exempt. The rule exempts graduates of schools in Australia, Canada (except Quebec), Ireland, New Zealand, the United Kingdom, and the United States. India is not on the list. A nurse educated in India therefore needs an English score for VisaScreen even if every lecture and textbook was in English, and even if a state board waived its own test.

The test list printed in the regulation is older. HHS sets the English standards, and TruMerit applies the current list published by the Health Resources and Services Administration (HRSA).

Which tests count for VisaScreen in 2026

HRSA updated the approved tests and passing scores effective May 12, 2026. According to TruMerit's statement, tests taken on or after that date are certified under the new criteria, and tests taken earlier are still judged under the previous ones. TOEIC is no longer accepted for tests after May 12, 2026. TruMerit also does not accept at-home versions of any English test, so book a sitting at a test center.

VisaScreen RN minimums for tests taken on or after May 12, 2026, as published by TruMerit. Check the live table before you book.

TestMinimum for registered nurses
Cambridge English (B2 First, C1 Advanced, or C2 Proficiency)169 overall; 194 speaking
TOEFL iBT4.5 overall; 4.5 speaking; 3.5 reading, listening, and writing
IELTS Academic6.5 overall; 7 speaking
Michigan English Test (MET)58 overall; 59 speaking
OET (Occupational English Test)360 speaking; 340 reading; 350 writing; 320 listening
Pearson PTE Academic55 overall; 76 speaking; 62 reading, writing, and listening

Two notes. The TOEFL iBT numbers use TOEFL's new scoring system, which TruMerit addressed in a January 2026 statement. And if you see a PTE speaking minimum of 63 quoted online, that figure belongs to TruMerit's earlier rules; the current table lists 76. The full table, including other professions, is on the VisaScreen page.

IELTS or OET? Both are accepted for VisaScreen. OET is built around healthcare communication, while IELTS Academic is a general academic test. Choose the one your board also accepts and whose format you can prepare for, because one result that satisfies both the board and VisaScreen saves you a second booking.

Confirm before you book a test

Test lists and scores change. This guide reflects the official pages as of October 2026. Before you pay for IELTS, OET, or any other test, confirm the current requirements with your state board of nursing and on TruMerit's VisaScreen page. Check how long each body accepts a score, too. For visa questions, speak with a qualified immigration attorney.

A sensible order for Indian-educated nurses

  1. Choose your state board. Read its rules for internationally educated applicants: whether it needs English scores before it makes you eligible, which tests it accepts, and at what scores.
  2. Pick one English test that satisfies both the board and VisaScreen. Where a board ties its scores to the federal minimums, as Maryland does, one result can cover both. Sit it at a test center.
  3. Start the credentials evaluation and board application. Maryland, for example, requires a foreign transcript to be evaluated by CGFNS or a successor organization. Paperwork takes time, so start early.
  4. Pass the NCLEX-RN. VisaScreen asks registered nurses to show they passed either the NCLEX-RN or TruMerit's Nurse Qualifying Exam, so your NCLEX result does double duty.
  5. Order VisaScreen with your English score, licenses, transcripts, and exam result when your employer or attorney says the visa process needs it.

While the paperwork moves, keep your NCLEX practice on its own calendar. How to Study for the NCLEX-RN lays out a weekly loop, and NCLEX-RN for internationally educated nurses covers the clinical-judgment habits that matter more than your English score on test day. Bottom line

The NCLEX-RN has no English test of its own. Your state board may require one before it makes you eligible, and VisaScreen requires one for nurses educated in India because India is not on the federal exempt list. For tests taken on or after May 12, 2026, the RN minimums include IELTS Academic 6.5 overall with 7 in speaking, or OET 360 speaking, 340 reading, 350 writing, and 320 listening.
